Congratulations, 2026-2027 LEAF Grantees!
- General Baptist State Convention of WI Inc. – H.O.P.E.
- This faith-based initiative will promote fall prevention through accessible health education, screening and preventive resources, and physical activity, with a focus on the Stepping On program in trusted faith and community settings.
- Curative Care Network
- Curative Care Network will launch a fall prevention initiative in Milwaukee and Waukesha counties by integrating all three CDC STEADI components into clinic protocols, offering free community education, and connecting participants with community resources. The long-term goal is to establish a comprehensive aging-in-place program at the clinic.
- University of Wisconsin – Stevens Point
- This project creates a referral pathway between the Stevens Point Fire Department, the Aging and Disability Resource Center of Portage County, and the University of Wisconsin– Stevens Point Doctor of Physical Therapy Program, connecting older adults who have mobility concerns to in-home assessments by supervised physical therapy students. Participants receive personalized recommendations, referrals, home modification guidance, or opportunities to try adaptive mobility equipment to help reduce fall risk.
- United Community Center
- United Community Center will host an interactive, culturally and linguistically responsive theater event “Negando la Gravedad” featuring older adult actors to raise awareness about fall prevention. The free event will engage Spanish-speaking older adults, families, and caregivers, particularly those facing barriers to traditional fall prevention education.
- CORE Community Resources
- CORE will expand instructor capacity for its evidence-based Stepping On and Ballroom Basics for Balance programs in northern Bayfield County, where isolation and transportation barriers can limit access to wellness programs. These classes will help older adults improve balance, build resilience, and strengthen social connections.
