Name: Safe at Home
Where: Dane County, WI
Major Project Activities: Safe at Home is a program offering free home safety assessments to Dane County older adults to prevent in home falls and accidents. The program is in collaboration with University of Wisconsin- Madison Occupational Therapy program, the Madison College Occupational Therapy Assistant program, and Safe Communities.
Safe at Home partners with students, retired occupational therapist volunteers or others who have a similar health care services background to provide assessments. At the assessment, each participant receives an Intervention Recommendations list, a folder with information regarding falls prevention, medication safety, and community resources, and a free night light. Following the assessments, the Safe at Home program coordinator provides follow up calls to participants to ensure that the recommendations are clearly conveyed and to provide connections to local services and resources.
