Zion Baptist Church (hosts); UW-Madison Pharmacy, Physical Therapy, and Nursing programs, Madison College Occupational Therapy Assistant program, and Edgewood College Nursing program (screenings and working with participants at both locations); UW Health and St. Mary’s (lunch); and the African American Health Network, the Dane County Falls Prevention Task Force, and the Community Academic Aging Research Network (CAARN) (contributions).
